Regional Medical Command
Advanced Life Support
personnel must file a medical command authorization application
with the council office annually for each service you wish to
provide ALS service to. This form is the only form accepted by
the PA DOH. Please print the form, complete it, attach the
appropriate copies and have your service medical director sign
it. Be sure to include copies of your official con ed
report, your EMT-P/PHRN card and your current CPR card.
The ALS practitioner must submit a new application with the
appropriate copies for each service he/she wishes to apply
for command with.
Medical Command forms are to be submitted to the council office
no later than January 31.
Your Continuing Education requirements must be met by December 31.
